Corrections

If something here is wrong, tell us. We record what changed and why, we date the correction on the page it affects, and we list it below. We do not edit quietly.

Recorded in the correction log

Corrections are treated as a priority. A page that has been corrected carries a dated note saying what changed, and the same entry appears in the log on this page. Nothing is edited quietly, because a reader who acted on the earlier version needs to be able to tell that it changed.

How to report an error

Every page ends with a link to report a problem with it, which carries the page reference for you. You do not need to know what is technically wrong — describe what you read, and what you think is inaccurate or out of date.

Report a correction

Opens the contact form with the topic already chosen. It is inactive in this build, and the page says so before you write anything.

What happens next

  1. The report is acknowledged within two working days.
  2. The Editorial Lead checks the claim against the sources the page records.
  3. A clinical claim is referred to the Clinical Governance Lead before any change is published.
  4. The page is corrected, dated and annotated, and the entry is added to the log below.
